To submit a claim in the first instance, we will require the following information:

• Policy number
• Name of Policyholder
• Policy Schedule and/or Certificate (where appropriate)
• Date of loss or discovery
• Loss location
• Cause of loss/Description of the incident
• Best contact with whom to investigate (Name, number, email)
• Estimated loss (if available)

Reporting a new claim by telephone is the preferred method and we require the following information:

• Policy number
• Name of Policyholder
• Policyholder driver name or last person in charge of the vehicle, with contact details
• Policy Schedule or Certificate
• Date of loss
• Accident location
• Accident circumstances
• Registrations of vehicles, involved, make, model, colour
• Damage caused to vehicles and any photos taken
• Third Party name, address and contact details (if any)
• Passenger details from both vehicles (if any)
• Witness details (if any)
• Police details (if any)

• Summary of claim/complaint
• Date of first awareness
• Date of claim made
• Documentation to support the notification including letter of claim, background documentation, correspondence, policy schedule and any other relevant documentation.
